5 Cities Homeless Coalition Awarded Portion of Dignity Grant By Justin Stoner

Homeless men and women who end up in the hospital but lack a place to stay and the resources to recuperate will get help through a grant awarded to 5 Cities Homeless Coalition thanks to Dignity Health Central Coast.
The coalition was one of many groups in San Luis Obispo and Santa Barbara counties last week awarded a portion of nearly $300,000 in funding. The grants are aimed at helping non-profit organizations with an interest in building healthier communities and caring for populations in need.
“This grant allows us to work with the hospital to identify who those individuals are; get them into respite care, but also work on those issues that are needed to stabilize them,” said Janna Nichols, executive director of the 5 Cities Homeless Coalition. “Sometimes the point at which someone becomes so medically fragile they enter the hospital, it’s an opportunity for a conversation to speak with them about services they may have not known about before.”
Dignity awarded grants under five project areas, including the Warm Hand Off Project, of which the coalition was a recipient. The Warm Hand Off Project seeks to reduce hospital visits from medically fragile patients, as well as help reduce mental health conditions and disruptive behavior for participants. Other project areas include the 20 Now Santa Maria program, the Diabetes Impact Group program, Expanding Access to Cancer Screenings and Primary Care in North SLO County program, and the Recuperative Care program.
The intent of the annual community grant is to help non-profit organizations with an interest in building healthier communities by improving health and living conditions. Criteria for this funding cycle were based on the following priority areas identified in the hospitals Community Benefits Report and Plan and Community Health Needs Assessment: Access to healthcare including mental health; Education; Homelessness/Housing; and Cardiovascular disease and stroke.
